What’s On Offer

Our team of specialized nurses provide many services such as education, annual reviews, referrals, advice and support with lifestyle management of diet, exercise, blood glucose testing, medication (including starting and adjusting insulin), smoking cessation, and weight loss.

Some of the free services available are:

  • 3 x 1/2 hour education sessions for newly diagnosed people with diabetes
  • Annual checks with the nurse like a ‘WOF’ for heart, eyes, kidney and foot health
  • For those who meet the criteria, there are quarterly visits for chronic care management for people with ongoing health conditions such as diabetes, congestive heart failure, respiratory disease, cardio-vascular disease and depression.
  • Eye testing and dietary advice with visiting specialists at the Practice
